The following documents are required to make a claim under the funeral cash plan:
- Policy document/Membership book;
- An official or certified copy of the original death certificate;
- A completed official claim form as prescribed by Botswana Funeral Services Group (Pty) Ltd t/a BAFA Funeral Services from time to time;
- Completed BI/DHA-1680 forms (if deceased died at home);
- Clearly legible certified ID copies of the deceased, Principal Member and beneficiary. New identity cards must be certified with copies of the front and the back of the identity card. In case of 3rd party payments, a certified copy of the 3rd party's ID document is required;
- Police statement and BI/DHA-1663, if death is due to unnatural causes;
- Bank details and a copy of a bank statement of the payee (or the third party in respect of third Party payments) for payment of the benefit;
In case for students proof from a recognized educational institute that the deceased is a full time student between the ages of 21- 25 (twenty one to twenty five) at the time the death occurred; and any additional documents that Botswana Funeral Services Group (Pty) Ltd t/a BAFA Funeral Services through its administrators and underwriters in its sole discretion deems necessary.
